Return to Exercise After Baby

Safe, structured guidance for returning to fitness after pregnancy — physical therapy to rebuild your foundation before ramping up intensity.

Getting Back to Exercise Safely

After having a baby, many women are eager to return to exercise — but the timeline and approach matter. Jumping back into high-impact workouts, heavy lifting, or intense ab work before your core and pelvic floor have adequately recovered can lead to injury, incontinence, or prolonged recovery.

Common signs that your body is not ready for your previous exercise routine include:

  • Leaking urine during running, jumping, or lifting
  • Pelvic pressure or heaviness during or after workouts
  • Abdominal doming or coning during core exercises
  • Low back or pelvic pain that worsens with activity
  • Feeling like your core is not engaging properly during exercise
  • Difficulty controlling movements that were previously easy

A postpartum return-to-exercise program bridges the gap between basic postpartum recovery and your fitness goals. Whether you want to get back to running, CrossFit, yoga, or simply feeling strong again, we help you build the foundation first so you can train confidently and safely.

Our Return-to-Exercise Approach

Our return-to-exercise program is structured around your specific fitness goals and current physical capacity. Treatment at The Back Clinic takes a progressive, milestone-based approach rather than following arbitrary timelines:

  • Baseline assessment — Evaluating your core function, pelvic floor strength, diastasis recti status, and overall fitness level to determine your starting point
  • Progressive loading plan — Gradually increasing exercise intensity based on your body’s demonstrated readiness, with clear criteria for each progression
  • Impact readiness testing — Specific clinical tests to determine when your pelvic floor can safely handle running, jumping, and high-impact activities
  • Core rebuilding sequence — Moving systematically from deep core activation through integrated full-body exercises that restore functional strength
  • Symptom monitoring education — Teaching you to recognize warning signs (leaking, pressure, pain, doming) that indicate you need to modify before continuing
  • Exercise program design — A customized home or gym program that progresses with you and accounts for the realities of life with a new baby

Every session at The Back Clinic is one-on-one with your physical therapist — never handed off to aides or shared with other patients. This individualized approach ensures your program advances at the right pace for your body, with real-time feedback on your form and technique during each exercise progression.

What to Expect

We typically recommend beginning a return-to-exercise assessment at six to eight weeks postpartum with your physician’s clearance, though every timeline is different. Whether you are eight weeks or two years postpartum, your therapist will assess where you are and build a plan that meets you there. Wear comfortable workout clothing to your first visit.

Most patients work through a four to eight week progressive program that takes them from foundational exercises to their target activities. The specific timeline depends on your delivery type, recovery progress, and fitness goals — a runner returning to half marathons will have different milestones than someone returning to yoga or strength training.

This program can run concurrently with pelvic floor therapy or as a standalone service for women who have already completed their core postpartum recovery and are ready to safely increase their activity level.

Most insurance plans, including Medicare, cover physical therapy for postpartum return to exercise. You do not need a referral to start treatment in Delaware. Call (302) 529-1900 or schedule online to book your evaluation.
