Post-Op Knee Rehabilitation

Expert knee surgery rehabilitation — ACL reconstruction, meniscus surgery, and total knee replacement recovery programs.

Knee Surgery Rehabilitation

Knee surgery rehabilitation is one of our most common post-surgical programs. Whether you have had an ACL reconstruction, meniscus repair or removal, total knee replacement, or another knee procedure, the quality of your rehabilitation directly determines your functional outcome.

Common knee surgeries that require post-operative physical therapy include:

  • ACL reconstruction — rebuilding the anterior cruciate ligament using a graft, requiring extensive neuromuscular retraining
  • Total or partial knee replacement — joint resurfacing for severe arthritis, focused on restoring range of motion and strength
  • Meniscus repair — suturing of torn cartilage, requiring weight-bearing and motion precautions during healing
  • Meniscectomy — partial removal of damaged meniscus tissue, with faster recovery but still requiring guided rehabilitation
  • PCL, MCL, or LCL repair — ligament reconstruction or repair for other knee ligament injuries
  • Patellar realignment or stabilization — surgical correction for recurrent kneecap dislocations

Each procedure has specific rehabilitation considerations and timelines. Your therapist will follow the protocol from your surgeon while adapting treatment to your individual progress, ensuring the best possible outcome from your surgery.

Our Post-Op Knee Program

Post-operative knee rehabilitation at The Back Clinic addresses the key challenges that determine your surgical outcome. Treatment is structured around evidence-based protocols specific to your procedure:

  • Early range of motion — Restoring full knee extension and progressive flexion is the top priority in the early weeks, as delays can lead to permanent stiffness
  • Quadriceps activation — Overcoming the muscle inhibition that naturally occurs after knee surgery through targeted activation exercises and neuromuscular electrical stimulation when appropriate
  • Swelling management — Techniques to control post-surgical inflammation that limits motion, muscle function, and recovery progress
  • Progressive strengthening — Building quadriceps, hamstring, and hip strength through a carefully progressed exercise program that respects tissue healing timelines
  • Gait normalization — Restoring a normal walking pattern as early as possible to prevent compensatory habits that can cause problems later
  • Balance and proprioception — Rebuilding the neuromuscular control that protects the knee during dynamic activities like stairs, uneven terrain, and sport
  • Return-to-sport testing — For athletes, objective functional testing including hop tests, strength symmetry, and agility measures to confirm readiness for competition

Every session at The Back Clinic is one-on-one with your physical therapist — never handed off to aides or shared with other patients. This hands-on, individualized approach is essential after knee surgery, where precise manual mobilization, real-time exercise feedback, and careful progression monitoring make the difference between a good outcome and a great one.

What to Expect

Physical therapy typically begins within the first week after surgery, and in some cases your surgeon may recommend a pre-surgical visit to prepare you for the recovery process. Early treatment frequency is usually two to three times per week, tapering as you progress and become more independent with your home program.

Rehabilitation timelines vary by procedure. Meniscus trimming may recover in six to eight weeks. Total knee replacement typically takes three to four months of active rehabilitation. ACL reconstruction requires six to nine months with formal return-to-sport testing before clearance for competition.

Your therapist will use objective measures — range of motion, strength testing, functional performance scores — to track your progress and guide advancement through protocol phases. You will always know where you stand in your recovery and what milestones are next.
